If you would like to save a few minutes at the office, we invite you to complete our new patient forms in advance. Once you have reserved an appointment time with us, we will text/email you a link where you can complete our forms online. Don't forget to smile before you upload your photo! Financial Options:
We are dedicated to taking care of members of our dental family here at the office of Dr Treva Lee in Fresno. Our caring, knowledgeable office team will guide you through the financial process so that you don't have to worry about a thing! If you have dental insurance, we can do a complimentary benefits analysis and file the insurance claims as a courtesy for you. We are always evaluating and adding new dental insurance networks and we are in-network providers for the companies listed below.

We participate with most of the major PPO Dental insurance networks. We are happy to do a complimentary benefits analysis so that our treatment estimates are as accurate as possible
Here are some of the dental networks that we have preferred provider status: Aetna, Ameritas, Assurant, Benefit Administrative Solutions, Blue Cross/Shield, Cigna, Cypress Benefit Administrators, Delta Dental, Dental Select, GEHA, Guardian, Healthcomp, Humana, MetLife, Principal, and United Concordia.
